Home Health Monitoring System

Using Lifelink Monitoring Services, we can monitor your vital signs and symptoms daily and take whatever actions are needed, including notifying you and/or your doctor of abnormal values. This system monitors your blood pressure, heart rate, weight and oxygen level all in your home with a simple ready to use system that electronically transmits information to our nurses. And the best thing about it is that there is no cost to you!

Who uses Home Health?

Many people do not know that home health care is an option or that Medicare pays 100% of home
health services for people who qualify. If someone has difficulty leaving their home without assistance,
then Home Health care is an excellent solution- The Home and Hospice Care Survey indicates that 7.2
million individuals receive home care services. About two thirds of them are over age 65. However,
people of all ages benefit from home health.

Many hospital patients are discharged to home care services for continued medical care. Others who
are managing chronic diseases, such as circulatory, heart disease or diabetes, use home health
care. It is also an excellent option for anyone rehabilitating from joint replacements or injuries
